Dear friend, I´m intending to visit my grandfather city in Poland and I´d like to know where would be possible there to make a survey about his documents: Książeczka Wojskowa, Dowód Osobisty, Wyciąg z Księgi Ludności or Poświadczenie Obywatelstwa Polskiego

such information you can get in city hall (Urząd Miasta), what should be in each city. Regarding Książeczka Wojskowa you should go to the local army centre - probably you will be able to get that office exact location information when you come to the city hall.
