Question
I've read several opinions, but are there definitive rules of formatting? Do I use a header? If so, does it go, title, author name, ch#, page#, and in that order? On every page?

Hi, John!

I get this question a lot, so I fully understand what you mean about "several opinions." To help aspiring authors out, I teamed up with a number of published authors to make a comprehensive list of the formatting do's and don'ts on a writer's forum I frequent. Hope this helps!
