Question If you use survey results in your book, do you have to have the participants sign a release form? If so where do you get such a form?

You ask an interesting question. I'm not clear from your question if the survey you ask about is one that you conducted.
If you conducted the survey, then you don't need releases from the participants. Their individual identities are hidden in the summary of your results. You also don't need to get permission to use the survey since you conducted it.
If someone else conducted the survey, then you don't need releases form the participants. You do, however, need permission to sue the survey results from the survey owner.
