As of the end of 2008 I will be certified for MRI as well as completing my Bachelors of Science in Radiographic Sciences (BSRS)degree. Currently I hold an Associates degree in Radiological Sciences from Kent State University in Ohio. I am a registered Radiographer in Ohio and Florida and certified by the American Registry of Radiologic Technologist (ARRT).

Subject: MRI

Question
I have low back pain for the last 10 years . Had two epidural injections twice . last one three years back . I had MRI on 21-06-08. Following are Radiologisit Observations from MRI . Do you suggest anysurgery ?

MRI of Lumbo Sacral Spine and screening Pelvis Performed

Protocol:  FSET2   - AXIAL
                 FSET1 & FSET2 - SAGITTAL


Screening : FSE T2 – SAGITTAL
Straightening of Lumbar Spine seen
Severe Degenerative End plate changes at L4/5 level
Rest of the Vertebral Bodies are normal in Height and Alignment
Anterior Marginal Osteophytes at L4/5 and L2/3 levels
Degenerated and dessicated  of all Lumbar intervertebral Discs noted
Reduced intervertebral disc height , disc dessication , diffuse disc bulge at L4/5 causing

   Bilateral moderate neuro-foraminal stenosis (Right > Left) abutting existing nerve roots
  
Disc Dessication , diffuse disc bulge at L3/4 causing

   Mild Interior thecal indentation
   Bilateral moderate to severe neuro-foraminal stenosis (Right>left)
Disc Dessication , Diffuse disc Bulge and postero central annular tear at L1/2 level
Mild Disc Dessication , left facetal arthropathy at L5/S1 causing
   Left Moderate neuro foraminal stenosis
Spinal Cord is normal in signal intensity

No Obvious abnormal pre and para vertebral soft Tissues
Spinal Canal Dimensions are normal ( L1=234, L2=243, L3=220. L4=226and L5=240mm sq)
Impressions: Findings suggestive of
      Degenerative Lumbar / Lumbosacral spine disease with  
Reduced Intervertebral Disc Height, disc dessication , diffuse disc bulge at L4/5 causing
   Bilateral moderate neuro foraminal stenosis(Right> Left) abutting existing nerve roots
     Disc Dessication , diffuse disc bulge at L3/4  causing
   Mild interior thecal indentation
   Bilateral moderate to severe neuro foraminal stenosis (Right > Left)
    Disc dessication , diffuse disc bulge and Potero Central Annular Tear at L1/2 level
     Mild Disc Dessication , left facetal arthropathy at L5/S1causing
   Left Moderate neuro foraminal stenosis
     (KINDLY CORRELATE CLINICALLY)

2) MRI PELVIS SCREENING
    
Small Divericula (1X1 cm approx) noted in right supero –lateral aspect of urinary      bladder .

No other obvious abnormality detected

You have a trashed back...but functional. There is NO guarantee that surgery would help you but it may.

If you want to avoid surgery have you considered pain management? It may improve the quality of your life...but you must excersise serious self-control because the meds are addictive and your body will come to depend on them.
