Question
What is required to convert and transfer my IRA funded with after tax money to my Roth IRA?

Answer
You will need to fill out a ROTH conversion form with your custodian and pay the tax.  The portion of your traditional IRA that is after tax, will no be taxed on the conversion.  Note - you cannot designate just that portion, it must be a percentage of the entire IRA.
