Question Hi, I have RA and OA. I am 50. Have been on Enbrel and ibuprofen. Past 2 weeks I have noticed increased knee crepitus and more difficulty climbing downstairs. Climbing upstairs has been a problem for a while. I do quadriceps exercises. What would you recommend as a next step? I have had one cortisone shot about 6 months ago. Should I try Synvisc?, should I see doc and ask for an MRI? Thank you.

All good questions and I'll address them in order.
The ibuprofen inhibits growth of new cartilage tissue and connective tissue and therefore serves only as a symptom modifier. This should be immediately discontinued for a number of reasons, least of which is kidney failure.
Glucosamine sulfate with MSM, an OTC supplement is in effect the same as the polio vaccine. When taken at therapeutic dosages, it rebuilds cartilage. This is well documented with countless research studies and should be taken at 3 gram/day.
Discontinue the quad exercises since this puts more wear and tear on the joints, swimming and walking briskly are the preferred alternatives.
Cortisone is very dangerous and provides nothing but temporary relief while also destroying vital tissues.
Synvisc is Hyaluronic Acid and has shown poor results long term and is painful and expensive.
Get an MRI and tell him you are going on GS with MSM for 6 months and repeat the MRI to see an astounding difference.
This is all well documented but apparently ignored due to capitalistic ambitions.
