you will find the connections you need at http://jizdnirady.atlas.cz/JRCis.asp?tt=c&cl=E5 . The site is in English, has the complete bus & train departure information and works for both Czech Republic and Slovakia.
I would go for "Combinations", but you can limit yourself to "Buses" or "Trains" if you wish.
In the form that opens up, all you need to fill is From, To, Date and Time.
Fill in "Praha" or "Prague" for Prague, "Strba" or "Poprad" for High Tatras (you choose), the others will do as you typed them.
