Question I am cross country runner and have been experiencing shin pain for the past three years. I have been to PT for it, have had multiple x-rays, an MRI and a bone scan to check for a stress fracture and nothing ever showed up. my PT originally told me my soleus was tight causing the pain. I have been doing stretching and strengthening and have custom orthotics as I overpronate but it does not seem to be working. I stopped running for two months to take a break from it and just recently started running. I have ran twice and already have started experiencing pain again. The first I ran half a mile and the second time I ran a mile. I am having medial left shin pain that is point tender with palpation, I also am experiencing pain with jumping as well. Could this be something besides shin splints? What should I do about this?
Answer Hi Ashleigh- sounds like you've really touched the right bases with physical therapy, X-Ray & MRI analysis, rest & orthotics. I've found that most persistant shin splint pain is foot related. Hopefully you're wearing your orthotics all the time & they were properly prescribed by podiatrist familiar with runners. You might need more rest & supervised return to walk-run activity by your therapist. Follow up X-Ray & MRI might also be indicated to see what if anything has changed. Make sure you are in your best substantial shoes, not minimalist or racing flats. live in the orthotics!
Try being a "fitness walker" for 3-4 weeks- & then retry gradual mixin of running. Keep up stretching & foot, ankle strength exercises & balance board work-
Have PT do soft tissue & ultra sound & consider another round of antiinflammatories. Hope this helps... Good luck, Dr.Bob Weil
