Question
Hello, Dr. Weil...
I am a 55-year-old 125 pound (5'2") weight female, who is a novice power walker/jogger.
I have osteoporosis and have been on oral meds for 13 years.  I noticed on occasion
I have pain in my big toe only when "pushing off" walking, and not every time...
kind of hard to explain.  It is sort of sharp.  I did some online research, and I feel it
is exactly in that MTP joint area.  I wear professionally obtained orthotics from a podiatrist
due to pronating (ankles roll inward).  My question is can this lead to a stress fracture?

Hi Laura,  doubt that you're dealing with possibility of stress fracture in big toe joint,(mpj). Have your podiatrist evaluate the problem area. Tendon or soft tissue inflammation could be problem. Early joint changes to cartilage, or the particular sesmoid bomes, under big toe joint could be involved. I would expect custom foot orthotics to be preventative & helpful so have them checked by podiatrist. Be a walker, NOT a jogger until you get evaluated. Having osteoporosis does make bones more susceptible to possible stress fracture so that possibility can also be ruled out. Pay attention to your shoes also regarding excessive wear.
