Kirk's alternate green wrap-around uniform was introduced in the episode "The Enemy Within" so that the audience would be able to tell the difference between the good Kirk and the evil Kirk.

I read the reason for the wraparound uniform was because it was difficult to get the original over Shatner's well-placed hair piece. The wraparound saved him time and annoyance before shooting a scene.

we just watched "the enemy within," and my 7 year-old daughter theorizes that they gave him the alternate uniform so we could tell the two kirks apart...it seems this was the first episode with the green uniform, so it makes sense...he also appears with his shirt off in the episode, and doesn't look all that fat, so i agree that it wasn't really a "fat shirt."

Also of note in "Charlie X" is the prototype of Kirk's wrap made of the same suede-like material as the first season equipment belts.  The character of Charlie Evans wears this wrap (with a pastel colored turtleneck under it) for the second half of the episode.

The wraparound tunic was originally created for "The Enemy Within" in order to make it easier for the audience to tell the two Kirks apart.
