Subject: no audio output

Question
I have a 65" JVC TV. I recently moved and can not figure out how to connect the audio. I have a digital cable box, the TV and a DVD/VCR. I have tried many combinations of connections but nothing works. Do I go from the cable box audio output to the tv input with the AV jacks or do I go from the DVD to the TV or ???  Also, I have a black square box on my screen and cannot get it off. It looks like it would be an information screen but there is no writing in it.

Devices are different but most probably this is the way to connect it:
AV wiring from cable box to input on TV
AV wiring from Combo VCR to second input on TV
The black menu box might disappear if you pree Display button on the TV remote (if there is one) otherwise check menu options.
