I can answer in regards to the highest earning, and some of the figures, however exactly how they earn it is somewhat confidential, and not all of that information is in the public domain, nor should it be, in my opinion.
Oprah Winfrey - 150.0 million pa TV Exec, Prod, Host, Endor...
David Letterman - 32.0 million pa
Howard Stern (radio) - 31 million pa - broadcaster...
