I have the same problem with mine too.  Box 9a & 9b both have the same dollar value on stock from "company T," which I've had more than 25 yrs after inheriting it from my father.  Dividend is never more than $100/yr.  I also got a 'nastygram letter' from the IRS, and it seems that I'm getting taxed twice on such a small amount of income.
