Subject: Varico cele
Question I have had previous occurence of Varicocele a year ago in October 2007, and i have had a surgery for it. I was feeling good and the sperm production was good, there was no discomfort.
But, Since 4 months, i have been doing too much physical labour like lifting weights in a store, and standing all day like 12 hours. So, now again, i feel almost the same kind of discomfort, i felt during a year ago. So, i am scared that, i am having Varicocele again. And i am only 24 years and yet to get married. I am planning to get married in another 2 or 3 years. So, i am scared of the infertility. What do you suggest about me undergoing an surgery again in another few months? or should i wait for a while?
And i have a habit of Masturbating very often, some times 2 or 3 times a week, so, is that a cause or is it going to effect my infertility during the time i have Varicocele?
Is it possible that i have a surgery again the second time, and then, if it again shows up, i have a surgery the Third time also? Or, is it not going to be possible for me to have a Surgery not more than twice?

Answer Tony, as you might imagine, for this type of problem it is not possible for me to make an exact diagnosis over the internet because of the inability to do a physical examination. However, I shall try to help you. There are many conditions that can give pain in this area. The most common are hernias and strain of the cremaster muscles. The testicles in the fetus are formed high up just below the kidneys and under the diaphragm in a space called the retroperitoneum. Just before birth, they begin to descend toward the scrotum. In order to reach this site, they must actually force a whole in the lower abdominal muscles. The spermatic cord supports the testicle(s) and contains the vas deferens, arteries, veins and nerves necessary for their function. Remnants of the abdominal muscles actually wrap around the cord as a permanent envelopment called the cremaster muscles. When one contracts the abdominal muscles, they simultaneously contract the cremaster muscles. The cremasters can be injured by strenuous physical activity. The pain is often transmitted to the testicles. Rest, heat, elevation, anti-inflammatory drugs and avoiding the activity leading to excessive strain are the treatment.
In most men, varicoceles are painless. Before you get too concerned about having varicocele surgery again, get checked by your urologist to see if the varicocele has recurred. i suspect is has not. More than 85-90% of varicoceles are cured by the first operation. In recurrent cases, surgery (or embolization techniques) can be used to cure the condition.
Masturbating a few times weekly will NOT adversely effect you fertility. I suggest you see your surgeon for re-evaluation. Most likely you are only suffering from a strain of the cremaster muscles. Good luck.
