Question
Hi - I've been suffering testicle pain for 2 yrs now - never both sides - usually starts on the left and moves to the right in the early evening.

A multitude of tests has been negative - and only a small grade 1 varicocele and cyst noted on the right (+ granuloma). I am convinced this is about venous congestion (relieved overnight and comes back 2 mins after getting up). My question:

Is it possible to get testicle pain due to venous congestion without evidence of marked varicocity?

While almost anything is medicine is possible, venous congestion is rarely the cause of any discomfort.  Without evidence of varicosities or varicoceles, you can pretty much rule out venous congestion as a source of pain.  

My advice is to get a second opinion from a urologist.
