Subject: Semen in urine
Question Hi,
I am 28 and living in Ireland. For the past 3 years or so I have had intermittent pain (a dull pain) in my scrotum. A couple of years ago I visited my Dr. and was referred to a urologist who diagnosed me with chronic epididymitis. However, despite numerous tests it was never established that I had a bacterial infection. Despite this I was given a few courses of antibiotics which did not have any effect. I am currently not on any medication for the chronic epididymitis although I still often have symptoms intermittently. I am sexually active and in a long term monogamous relationship with the same partner for 7 years. I don’t believe that I could have a STD.
The new problem is that on two occasions in the past few days I have found that following using the toilet semen comes out of my penis. When I stand up after use of the toilet I feel a slight urge to pass more urine and when I do, I pass a small amount of urine with semen in it.
Also, my semen always (following regular ejaculation) contained lumpy globular sticky bits. Is this a normal consistency for semen?
I would really appreciate any advice/information you might have.

I doubt that what you are seeing is actually semen. Prostatic secretions or a urethral discharge are more likely. At the end of urination, the prostate squeezes shut. This can express a small amount of prostatic fluid which would look like semen. The only way to express semen is to squeeze the bulbocavernosus muscle and put pressure on the seminal vesicles. This does not happen during voiding.
Semen is normally sticky and has globs. Whether yours is within the usual, normal range or not I can't tell.
Your chronic epididymitis needs to be looked into more. A testicular ultrasound and possibly even a surgical exploration may be needed to do this. Talk to your urologist or consider a second urology opinion.
