Question
Hello Dr. Leslie,

I am a 29 yr old sexually active male. I have a lump st the bottom my left testicle which I found out one fine morning almost 2 years back. It was a huge swelling and it was very painful. During that time(initial 3-4 weeks) there used to be some off-white sticky fluid discharge from my penis. And the penile shaft used to itch a lot.
Gradually the size of the lump has reduced and has settled to a size of a peanut which was as big as a ping-pong ball. It is hard and heavy. But the pain is still unbearable every time it get squeezed slightly. And the right testicle also feels heavier than before and appears to be swollen a bit on the top or have been displaced upward a bit.
Off late I have been observing some sort of white substance being accumulated(pungent smell) on my penis and the rim of the penis head also itches sometimes and sometimes there are small red spots around the area. Though I am not circumcised, I always keep the area clean.
And for last few days I have been experiencing slight pain near left groin area.
I don't have any sexual dysfunction or pain during sexual activities. Also, I never experienced any bleeding from penis. But I feel slight burning sensation sometimes during urinate, which I feel is negligible.
Earlier I consulted an Urologist who told me that this is nothing but scrotal cyst. So there is nothing to be worried about(Ultrasound was not performed).
Could you please help me on this matter?

Answer
If the urologist determine this was a scrotal cyst, he was probably correct even without the ultrasound.  But this owuld not explain the white discharge which sounds like an infection. I suggest you consult a different urologist and get a better diagnosis.
