Urology - knowledge on Stricture Urethra disease

Question QUESTION: I am a 50 yrs old Pakistani man, facing urinary retention problems since 20 yrs, Urologist of Pakistani Hospital diagnosed as Stricture Urethra,lot of dilatation done, lot of Cystoscopy was don, 06 or 08 time Optical urehtrotomy was done, and now 25th Feb 2009 Buckle lay Urethroplastee was also done but i am still in urinary retention problems what to do now ??
i am very tired with this disease, please help me guide me,

This is obviously a complicated issue. A urethroplasty is a major repair for a stricture and is usually quite successful. At this point, the reason for the failure needs to be determined.
The world leaders in this kind of problem are in Norfold VA in the US but there may be other centers nearer to you.

QUESTION: Sir i wish to know in details of cause of failure of this major operation and further more how to be sucessfull with future operation, what to do for suceesfull,which procedure will be now sweatanle for me,

What you ask is beyond the responsibilities of this forum. However, in short, possibly causes for failure include infection, disruption of suture line, leakage, wound dehiscence, fistula formation, scarring, devascularization of the mobilized tissue, etc. Lots and lots of possible reasons for failure which is why urethroplasties are typically done only in major centers that do them frequently. Proper technique helps but there is no way to guarantee a good outcome.

I can't tell you what the optimal treatment should be because the exact nature of your remaining problem is unclear. Talk to your physicians about possible solutions. Perhaps a visual urethrotomy or dilation might be sufficient or possibly they might suggest that you do a daily stretching of the stricture. Or this might do more harm than good. You need specific advice that will depend on the nature, location and severity of the remaining problem. Consider getting a second opinion from another medical center.
