Question

Dear Dr.

Thanks for your immediate response.  I have another matter to discuss with you.  I am taking Hytrin 2 mg daily at bed time since a month. There is a bit of relief in urination. I have to wake twice for night urination. A couple of days before I also started taking Homeopathic medicine "PROSTAKAN" which containes 160 mg of herbal extract Saw Palmato. I take two capsules 1+1 daily. Yesterday I experienced burning sensation on the tip of pinus.  I was told by my health care provide this is due to some inflamation of the liver and was advised to drink soda mixed with milk.  I did so and it really worked.  However, I am also feeling feverish condition since tomorrow although I have stoped taking the homeopathic medicine. What is your opinion in this matter.  Should I continue both the medicines or leave the one for better treatment?
Looking forward your positive response.

-----Question-----
I would like to ask if there are any medicine without serious side effect, which can shrink the prostate. I am told that Proscar or Saw Palmeto are very sueful.  What is your learned and expertise advice on these medicines?

Thanks.
Bilal
-----Answer-----
Bilal, i have already written to you that Proscar and saw palmetto are medicinal options for treating BPH.  All drugs have potential side effects and therefore the risk benefit ratio has to be discussed with your urologist.  Saw palmetto is a natural herbal product with fewer side effects.  It can be purchased over-the-counter.  The usual daily dose is 320 mgm.  The exact mechanism of action is not know but it does seem to relieve symptoms in about half the men who take it for at least 3-6 months. Proscar (and Avadart)  are relatively expensive prescription medications that actually shrink the prostate gland over a period of 6 months.  They work best on larger prostate glands.  Side effects can include breast tenderness or enlargement, impotence, decreased ejaculate volume, allergic reactions, etc.  As with all these medications, if you get relief and discontinue the drug, the symptoms will recur.  I have previously sent you all the treatment options for BPH.  You need to discuss these with your urologist.  Good luck.

Answer
Bilal, burning at the tip of the penis and fever suggest an inflammation in the urinary tract and not a liver problem.  It is highly unlikely that your medications are responsible for this and I see no reason to stop them if they are helping your voiding symptoms.  The most common cause of urinary inflammation in a man is prostatitis, which often is due to an infection in the prostate gland.  If your symptoms persist, I suggest you see your urologist for evaluation and treatment.  Good luck.
