Question My boyfriend has a problem starting his stream of urine and is slightly painful, but mostly during sex he gains a erection, and when he feels like he needs to ejaculate a hard knot forms on one side of his penis, then the knot gets mushy and moves back down the shaft but he feels as if hes ejaculated but nothing comes out. He is almost 40 yrs old and we do have a very active sex life.
Answer Voiding problems with the stream are relatively common. Prostatitis would be most common in this age group. I can't explain the "knot" except that it could represent a urethral diverticulum together with retrograde ejaculation. This would be exceedingly rare but not impossible. He should consult a urologist for a definitive answer.
