I thought I'd chime in, since I've read quite a few suggestions, and we're going to do it a bit differently.

In our wedding, the bride's mother died when the bride was 7, the bride's father remarried soon after.  The new step-mom is very much a part of the family, and the bride considers her to be her mom (in addition to her deceased mother).  The bride wants both mothers to be on the invite.  The wording of our invites:

Mr. and Mrs. Bride's Father

And on behalf of the late Bride's Mother

request the honour of your presence at the wedding of their daughtor

The Bride

to

The Groom

Son of Mr. and Mrs. Groom's Father

at the wedding time

on the wedding date

at the wedding location
